New Instructions for Completing Form FRA F6180.54- Rail Equipment Accident/Incident Report

As of December 5, 2023, the instructions for completing Form FRA F6180.54- Rail Equipment Accident/Incident Report have been modified to gather additional data concerning train length and number of crew members who were aboard the controlling locomotive at the time of the accident. These new reporting requirements are required by the Infrastructure Investment and Jobs Act (IIJA) Section 22421  and will help FRA to better address risks to railroad safety and the general public.

For a three-year period, from December 5, 2023 through December 31, 2026, FRA will utilize the Form’s Special Study Blocks 49a and 49b to collect the following information regarding trains involved in FRA reportable accidents:

  • Block 49a- the length of the involved trains, in feet, and 
  • Block 49b- the number of crew members who were aboard a controlling locomotive involved in an accident at the time of such accident

All previous data collections that utilized Special Study Blocks 49a and 49b have been replaced by this collection, effective immediately.

All current instructions for completing Form FRA F6180.54 remain unchanged.
